docs: complete Code of Conduct enforcement guidelines

The Code of Conduct was missing enforcement guidelines sections 2-4.
Added the missing sections from the Contributor Covenant v2.1:
- 2. Warning
- 3. Temporary Ban
- 4. Permanent Ban

This ensures the document matches the standard Contributor Covenant template
and provides clear guidance for community leaders on handling violations.
