fix: diagnose next-step suggestions — planning and shipping, not investigate

Replace /investigate suggestions with actionable next steps:
- Simple fix → implement it, then /review + /ship
- Complex fix → plan + /plan-eng-review
- Scope question → plan + /plan-ceo-review
- Fix PR ready → /review + /ship
