feat: add experimental native chat connectors (#13038)
## Thinking Path

## Linked Issues or Issue Description

Refs #13100 and #13092. This is the second of exactly two chat PRs.
Foundation #13100 is merged and changed 143 files. Runner prerequisite
#13092 is also merged. This PR changes 400 files against master, below
the 500-file review limit. It contains no wireframe images or HTML
galleries.

## What Changed

- Add native Slack, GitHub, Microsoft Teams, Telegram, and Discord chat
connections. Keep chat disabled unless the operator enables experimental
chat connectors. Preserve the production GitHub tool connection and its
normal setup path.
- Bind each provider bot identity to one immutable Paperclip agent. Bind
each admitted external conversation to one task. Paperclip owns tasks,
runs, permissions, and audit records.
- Add durable admission, per-conversation queues, questions, task
controls, progress, final replies, images, files, and delivery receipts.
Board comments remain internal unless explicitly sent to the channel.
- Check current identity, provider reach, resource access, credentials,
runtime generation, and exact source before provider effects. Keep
private responses private. Never send raw reasoning, private logs,
credentials, or tool arguments.
- Hold uncertain sends for explicit audited resolution. Make Board
Send-to-channel atomic and idempotent. Keep reconnect and setup
credentials in Paperclip secret storage.
- Preserve current native-runner authority across retries, lost
acknowledgements, and recovery. Keep immutable input and completion
contracts separate from newer user input. Receipt reconciliation cannot
launch a provider.
- Reconcile chat close/new ordering and provider-effect lock order.
Audit resource access changes in the same transaction. Submit only the
selected resource from each UI toggle so stale pages cannot undo
unrelated access changes.
- Drain Codex stdout before certifying process exit. Bound the drain
with the existing shutdown grace. Preserve observed terminal authority
without treating an undrained process as successful or reusable.
- Incorporate master `018ca5da` with its ACP Stop, mobile task layout,
runner packaging, and official lock changes. Preserve dedicated
chat-answer continuations in both directions when ordinary queued
comments are adopted after Stop.
- Fence late adapter readiness behind an earlier Stop for the same run.
Preserve verified cleanup for registered adapters. Handle single Stop,
agent pause, duplicate Stops, and failure release without creating a
false cancellation receipt.
- Incorporate master's `6dd48cad4` wake-queue extraction. Preserve exact
failed-chat retry authorization and lineage, retired question-source
suppression, and the block on generic recovery that would discard the
admitted source. Fresh deferred input retains its separate promotion
path.
- Incorporate master `2a05b5ed3` and its queue-admission extraction,
simplified transaction ports, and separate runner CI job. Preserve exact
durable receipts, actor separation, and dedicated-answer isolation
through the new module. A failed receipt insert rolls back the
accompanying deferred-wake merge.

## Verification

Current head: `afe19299d06253cb628eb398e91d1200ea9f412a`, incorporating
master `2a05b5ed3457ea33efd6895520447d1d97fe98d8`. The conflicts are
resolved. This successor fixes two test-harness boundaries exposed by
CI: per-case route-module preparation and actual durable-save completion
before intentional runner termination. Production code and all existing
test/turn deadlines are unchanged. [Exact-head Greptile
review](https://github.com/paperclipai/paperclip/pull/13038#issuecomment-5587250594)
is **5/5**, completed September 10 at 13:20:55 UTC, with no actionable
findings or open review threads. [Fresh exact-head
CI](https://github.com/paperclipai/paperclip/actions/runs/34481724341)
passes **all 24 jobs**, including Build and both required aggregates.
Normal exact-head guarded merge was attempted and rejected by the
remaining branch approval policy: CODEOWNER review is required and no
human approval is present. Normal **squash auto-merge is enabled** as of
September 10 at 13:36:26 UTC. Requested CODEOWNERS have been notified;
no approval bypass or self-approval was used. Earlier-head results below
remain historical evidence, not qualification of this successor.

- Final exact-head Linux evidence: 995/995 chat integration cases; 36/36
agent-skills routes; 35/35 runner live-session cases, including real
process kill/resume; 1948 runner Vitest cases with three existing
benchmark/platform guards; 870/870 API-authority cases; and 104 browser
cases with four existing optional skips. Rust, conformance/replay, full
repository build, typecheck, canary, all server/workspace shards, and
both required aggregates pass with normal CI concurrency. Earlier failed
attempts remain recorded below.

- Latest test-only qualification: 141/141
route/permissions/authentication cases pass in separate cold forks, with
plain server types and independent review clear. The real-runner suite
passes 35/35, with plain runner types and independent review clear. A
controlled premature-save acknowledgement fails as expected; matching
ownership/effect/process evidence, rejected saves, real turn outcome,
test abort, and pre-kill liveness are covered. No local reproduction of
the original CI scheduling failure is claimed. The preceding [CI
run](https://github.com/paperclipai/paperclip/actions/runs/34479680858)
passes 21/24 jobs, including all 995 Linux chat cases and browser
aggregate (104 passed, four existing optional skips); only Build, the
skills serialized shard, and the required verification aggregate fail.
Its exact-head Greptile review was 5/5. Both failed job logs are
retained.

- Final fixture qualification: all eight focused Discord cases and all
995 chat integration cases pass. The exact modal statement/PID is
observed before taking the real connection lock; the test then proves
its actual blocking relationship before mutation. Original SQL
execution, provider behavior, negative assertions, and 1s/15s timeouts
remain unchanged. Independent review is clear and test/production hashes
remain frozen. The preceding [CI
attempt](https://github.com/paperclipai/paperclip/actions/runs/34477184777)
passed 22 jobs, including Build/runner, typecheck, canary, all other
test shards, and browser aggregate (104 passed, four existing optional
skips); the two fixture failures and failed verification aggregate
remain recorded, not relabeled as a pass.

- Current queue-module composition: 308/308 recovery/batching/queue/Stop
tests; 995/995 full chat integration; 89/89 module tests, including real
PostgreSQL receipt-insert rollback; 24/24 workflow/module-boundary
tests; plain server and UI types. All four actual local process/ACP
browser paths pass in 1.4 minutes. Fresh databases, no skips or retries,
stable reviewed source hashes. The initial boundary failure is retained;
its no-op service wrapper was removed without changing recovery context
or weakening the check. An exploratory standalone test-directory
typecheck fails because its new upstream transformation config is not a
standalone typechecking project; standard CI/build does not invoke it,
and no configuration was weakened to suppress those diagnostics.

- The preceding head `e02a63d462ce5d47433b0aeb632bb6fd20aab1ba` passed
[all 24 CI
jobs](https://github.com/paperclipai/paperclip/actions/runs/34436462958)
and exact-head Greptile review at 5/5. Required CODEOWNER review
prevented its normal merge before master advanced again.

- Final extracted-module composition: 307/307 recovery, batching, queue
and Stop-control tests; 995/995 full chat integration; 49/49 module
tests including eight PostgreSQL adapter cases; and 19/19 issue-update
tests. Plain server types pass. All four actual local process/ACP
browser paths pass in 1.3 minutes. Fresh databases, no skips or retries
in these cohorts, frozen source hashes, and independent review clear.

- The preceding head `3e4e1c1c` passes [all PR CI
jobs](https://github.com/paperclipai/paperclip/actions/runs/34415826820),
including Build and required `ci / verify` and `ci / e2e`. Both the
original Rust failure and the previously load-sensitive lineage fixture
pass with unchanged Linux concurrency. Master advanced afterward and
required this reconciliation.
- Final master composition: 448/448 focused UI tests, 186/186 adapter
tests, 24/24 queue/control tests, and 11/11 packaging tests. Plain UI,
server, shared, and adapter types pass. Token gates and diff checks
pass. Independent server and UI reviews are clear.
- Stop-registration regression: both real-service cases fail against
exact `a95` source and pass with the fix. The full corrected
recovery/control suite passes 265/265. Duplicate-owner and failed-Stop
controls also pass. Plain server types pass. The readiness barrier
prevents provider startup without adding an acknowledgment to an already
terminal run.
- Final qualification strengthens terminal-field equality and repeats
both affected cases successfully on a fresh database. All four actual
local process/ACP browser paths pass again in 1.3 minutes, without skips
or retries. The final screenshot shows Cancelled, a paused subtree,
retained input, and no error toast.
- Two new actual-service regressions fail before the merge fix. They
prove that queued-comment adoption could consume a dedicated chat answer
or add unrelated input to that answer. The fixed four-case cohort
passes, including ordinary upstream continuation and adapter Stop
controls. Full recovery passes 257/257. All four actual local
process/ACP Stop browser flows pass in 1.4 minutes, without skips or
retries, on a fresh database.
- The unchanged runner artifact was qualified with 171/171 transport
tests, 870/870 API-authority tests, conformance 1/1, and replay 11/11.
Six controlled reader tests prove the exit/drain repair. Its local
serial Rust workspace passed 546 top-level cases plus two invoked
helpers; the later passing Linux CI supplies default-concurrency
evidence.
- Prior exact-source full chat integration passes 995/995. Settings
regressions cover concurrent stale pages, 501 destinations, pending
state, rejected updates, and explicit retry. These deterministic tests
do not prove live provider behavior.
- Retained failed attempts and their causes are in the [qualification
log](afe19299d0/doc/plans/chat-adapters/2026-09-08-chat-queue-and-webhook-repair.md).
The first merge adapter run timed out while macOS slept for 290 seconds.
Its unchanged repeat passed with a temporary sleep guard. No assertion,
deadline, or CI gate was weakened.

Review commands include `pnpm --filter @paperclipai/server exec vitest
run src/__tests__/heartbeat-process-recovery.test.ts
src/__tests__/issue-queued-comments-routes.test.ts` and `pnpm exec
playwright test --config tests/e2e/playwright.config.ts
tests/e2e/acp-stop-continuation.spec.ts`. Database suites require fresh
disposable databases. See the [browser

## Risks

- This remains experimental. Deterministic tests and bounded live
evidence do not establish every provider feature, tenant, permission
layout, or media shape. Teams work-tenant qualification is still open.
- Failed and uncertain provider effects remain visible and can require
operator action. A transport receipt does not prove recipient
visibility.
- Native controller and runner artifacts must remain compatible.
Preserve lease ownership, terminal authority, source binding, and
quarantine during future changes.
- Access and audit rows commit together, but activity notifications
remain best-effort. This is not a new durable event outbox.
- The PR operation does not deploy a live server, replace its runner, or
change provider permissions. Remaining live qualification is documented
in the [temporary
handoff](afe19299d0/doc/plans/chat-adapters/2026-09-08-open-qualification-followups.md).

## Model Used

OpenAI Codex assisted with implementation, tool execution, testing, and
review. The work records `gpt-6-astra` assistance. The environment does
not report a context-window size. No private reasoning traces are
included.

Make task recovery durable and preserve the latest user request across native and legacy continuations. Keep routine recovery quiet and prevent replay when action outcomes are uncertain.

feat: review connection actions from tasks (#13063)
Bring governed connection reviews into task history and composer approvals. Share resolution with Connections, add scoped remembered permissions, and resume agents through durable outcome receipts.

Keep cards compact, collapse raw results, isolate untrusted provider output, bound continuation payloads, and reconcile missed live events. Add Storybook coverage, browser journeys, and service regression tests.

Verification: all PR CI gates passed, Greptile 5/5, security scans passed, five connection-review browser journeys passed, and real native Codex approval/continuation was verified against the local MCP fixture.

fix: repair runner configuration, macOS execution, and artifact galleries (#13062)

## Linked Issues or Issue Description

**What happened?**

Converting an existing Claude agent to Paperclip Runner failed with a
Codex-only restriction. ACPX Claude showed unrelated models and required
`claude-sonnet-5`. Its native runtime rejected macOS. Configuration
mixed common model settings with process controls. Artifact cards
labeled “Open gallery” navigated to attachment URLs instead of opening
the task gallery.

**Expected behavior**

Conversion keeps agent identity and compatible settings. ACPX Claude
uses the normal Claude catalog and accepts typed model IDs. Codex uses
the native runner. The verified Claude runtime can launch on macOS ARM64
and x64. Common configuration sections place the same fields together
across adapters. Artifact images open in the shared task gallery with
navigation and downloads.

**Steps to reproduce**

1. Open the configuration of an existing Claude agent.
2. Convert it to Paperclip Runner.
3. Select ACPX Claude and a different catalog model or a typed model ID.
4. Save the agent and run a disposable task on macOS.
5. Inspect configuration and advanced run-policy controls across
adapters.

**Paperclip version or commit**

The bugs were reproduced on `165ca56a22adb60e5fda56045442d9c8498116a8`.
This branch was rebased onto `7ed122911`.

**Deployment mode**

Built from source. Local test-drive instance on macOS ARM64 with an
isolated database.

Related work: #11798 addresses unsupported ACP session options in the
existing adapter path. #13048 addresses working-folder preservation.
This change fixes native runner configuration and launch behavior.

## What Changed

- Remove the Codex-only conversion restriction. Preserve agent identity,
instructions, directories, credentials, and compatible model settings.
Reset incompatible sessions while retaining history.
- Show ACPX Claude and native Codex as distinct provider choices. Remove
ACPX Codex from advertised configuration. Normalize legacy
configurations before fresh runs without rewriting historical run
descriptors.
- Select model catalogs and cache entries by provider. Support refresh
and typed model IDs. Pass exact Claude IDs through session creation,
model changes, and recovery.
- Add verified macOS ARM64 and x64 Claude SDK snapshots. Bound
executable allocation and total snapshot size. Preserve package checks,
dependency isolation, process ownership, cancellation, and Linux
descriptor loading.
- Probe local runtime readiness. Report remote platform checks as
incomplete until the remote runner verifies its runtime.
- Surface actual model rejection and allow correction and retry.
- Repair missing ACPX goal-capability helpers exposed by the post-rebase
live test. Persist and restore the optional capability without breaking
session startup.
- Put Agent identity first and intentionally remove the Capabilities
editor, as requested. This is removal of UI editing, not relocation:
preserve existing capability metadata and API compatibility without
adding another editor. Use the themed select for configurable permission
modes, with normal text instead of monospace.
- Put model and provider under Adapter. Give environment variables their
own section. Fold command and arguments under Configuration. Fold
lifecycle, timeout, and interrupt grace under Advanced Run Policy. Hide
single-option permission controls.

- Open image and video artifact cards in the existing task gallery,
including cards in the artifacts panel. Chat attachment images use the
same gallery. Preserve standalone media previews and download links.

## Verification

- Rebased focused UI/API/database suites: 293 tests passed.
- Rebased native runtime and ACPX suites: 242 passed, 7 skipped.
- Repository typecheck, build, and token gates passed for the runner
changes. Gallery follow-up UI typecheck, build, and token gates also
passed.
- Follow-up UI suites passed (86 tests), packaging checks passed (14
tests), and the final focused runtime suites passed (126 passed, 7
skipped).
- Linux container isolation and lifecycle fixtures passed before rebase
(57 passed, 2 skipped). Rust ACPX provider-session tests passed after
rebase (8 tests).
- Browser tests completed actual Claude and native Codex tasks on macOS
ARM64. They covered conversion, catalog refresh, a non-default catalog
model, a typed `haiku` ID, save/reload, cancel, follow-up session
continuity, invalid-model errors, and recovery.
- Final-revision live tests completed a typed Claude task, a follow-up
with the same provider session, and a native Codex task on macOS ARM64.
- Browser tests confirmed the moved interrupt-grace field saves and
survives reload. Cross-adapter tests cover Claude, Codex, Gemini,
process, gateway, and schema forms.
- Full local run: 7,080 passed, 30 skipped, and two timeouts. Both
timeout suites passed on isolated rerun (84 tests); the failures were
the plugin login-worker exit diagnostic and the runner real-server
vertical slice.
- Final follow-up checks: 50 registry tests and 45 snapshot/installation
tests passed (6 platform-specific skips). Oversized executable rejection
is covered before allocation or reading; unsupported-platform tests
invoke the real installation probe.
- Runner head `ddb5101c483a297f74875ab96b3c66035b002d50`: all CI gates
green, including full runner verification, repository build, typecheck,
general/serialized server suites, browser tests, and canary dry run. [CI
run](https://github.com/paperclipai/paperclip/actions/runs/34286178670).
- Greptile: 5/5 on that runner head. All four review threads resolved.
Superagent, Socket, and Snyk checks green.
- After snapshot hardening, another real Claude task completed on this
Mac using the rebuilt runtime.

- Gallery follow-up: 148 focused tests passed, covering artifact
selection, shared attachment collections, deduplication, image/video
cards, standalone previews, downloads, and closing. Live browser
verification completed on the settings follow-up: artifact selection,
6-image pagination with wrapping, download action, and closing all
stayed on the same task URL. All checks passed on gallery head
`96136da58ff195bf6ca00b281eb3022ad12d7bd8`: [CI
run](https://github.com/paperclipai/paperclip/actions/runs/34287987536).
Greptile returned 5/5 on that exact head with no unresolved threads.

- Final settings polish: 96 focused tests, UI typecheck/build, and token
gates passed. A real browser walkthrough verified readable permission
options, identity placement, Capabilities removal, and permission
save/reload. Original test-agent permission mode restored. All 31 checks
passed on final head `e46540d6bf32bfb0566dca16b2f4a75ba437618c`: [CI
run](https://github.com/paperclipai/paperclip/actions/runs/34292797886).
Greptile returned 5/5 with no unresolved threads.

## Risks

- Capabilities intentionally has no editable UI field after this change.
Existing values remain readable and API-compatible; removing the field
does not erase stored metadata.

- macOS launch now copies verified package files into private snapshots.
The implementation must retain isolation and clean up snapshots on exit.
- Runtime provider or model changes reset the current session.
Historical runs remain available.
- The macOS x64 SDK executable digest was verified, but a live Intel Mac
run was not available. Linux verification used container fixtures, not a
real Claude task.
- Remote environment tests report a warning when only the platform has
been checked. They do not claim package readiness from the server host.

## Model Used

OpenAI Codex, based on GPT-6. The exact served model identifier and
context-window limit are not exposed in this session. Used reasoning,
repository inspection, code execution, Rust and TypeScript tests, and
browser automation.

fix(runner): restore local session and task integrity (#12721)
## Thinking Path

qualified provider-session continuity, and restores one lossless

## Linked Issues or Issue Description

**What happened?**

A second native heartbeat could reuse tickets, leases, command receipts,
sequence state, and run identity from the first heartbeat. Provider
phase and item identity could be lost before the UI read them. Redaction
could corrupt protocol discriminators while still missing malformed
credential tails. The task thread could fold progress into the final
response, hide failures, or show more than one final answer. Native
Codex also exposed approval modes that do not yet have a durable
approval bridge.

**Expected behavior**

Each heartbeat uses a new PRP authority epoch. Codex and OpenCode
preserve exact qualified provider sessions; ACPX emits an explicit
continuity event when its qualified process-replacement policy is used.
Every accepted provider event is presented, classified as internal, or
surfaced as unsupported. The task page shows chronological progress,
reasoning summaries, activity, Plans, interactions, terminal failures,
and exactly one final reply. Direct adapters retain their existing path.

**Steps to reproduce**

1. Enable the unified experimental Paperclip Runner setting.
2. Create a local native Codex, OpenCode, ACPX Claude, or ACPX Codex
agent.
3. Run response, Plan, structured-question/resume, restart,
cancellation, and failure scenarios.
4. Reload the task while active, waiting, failed, and settled.
5. On the old implementation, observe stale run authority, missing
classifications, incomplete output, or duplicated/folded replies.

**Paperclip version or commit**

The repair is based directly on `master` at
`87d05e194b643810d16d20612115acd01d735d43`.

**Deployment mode**

Local development with the embedded database.

Related work: Refs #12616, #12646, #12666, #12685, and #12700.

## What Changed

- Rotates PRP control-plane, outbox, ticket, lease, command, receipt,
and sequence authority for each heartbeat while carrying forward only a
validated provider-session identity.
- Reads `control-plane-state.json`, validates both durable schemas and
lifecycle values, resumes coherent current runs, archives qualified
settled authority, and quarantines malformed or mismatched scoped state
without moving ambiguous live legacy state.
- Preserves Codex provider phase and stable item identities so
commentary remains progress and only `final_answer` becomes final.
- Adds raw OpenCode HTTP/SSE boundary coverage and canonical reasoning
lifecycle mapping.
- Makes ACPX normalization lossless for visible reasoning, tool

The already-landed runner package has a qualified Codex path, but the
application needs durable native-run state, guarded runtime selection,
authenticated coordination, tool security, finalization, and recovery
before the experimental adapter can be exercised safely.

Add a Codex-only `paperclip_runner` application path behind the existing
default-off native-runner setting. Bind native state and coordination to

A skill-capable legacy local agent could start without

stored skill preference. An explicit empty preference also removed the
skill. The agent then reported that the Paperclip skill was not
available.

**Expected behavior**

Every skill-capable legacy local adapter must mount the Paperclip
operational skill when the runtime inventory contains it. Optional
skills must remain configurable. The native runner must keep its current
protocol-based behavior.

**Steps to reproduce**

1. Create a non-CEO `codex_local` agent without `paperclipSkillSync`
preferences.
2. Start a legacy heartbeat.
3. Inspect the managed `CODEX_HOME/skills` directory.
4. Observe that the Paperclip skill is absent before this change.

**Paperclip version or commit**

The problem reproduces on `master` before this pull request. PR #7029
introduced the configured-only selection behavior.

**Deployment mode**

Local development and self-hosted legacy local adapters.

`/private/var/...`. The changed adapter suites pass independently.

- This change deliberately makes the operational skill non-removable for
skill-capable legacy local adapters.
- Existing agents receive the skill on their next list, sync, or run
boundary. No database migration is required.
- The resolver does not create a skill when the runtime inventory does
not contain the canonical entry.
- Hermes aborts a run if another installation occupies the required
operational skill target.
- Hermes removes only an undesired Paperclip-owned symlink that still
points to the known Paperclip source.
- The native runner does not receive the legacy default.

generic "not available" with no cause. On claude-local, a desired skill
whose source path does not exist reaches the prompt-bundle hasher, whose
`fs.lstat` throws and can fail the whole run.

**Expected behavior**

The skill appears with `sourceStatus: "missing"` and a `missingDetail`
carrying the underlying error, snapshots and the UI show it as broken,
and adapters skip it at mount time with a logged warning instead of
crashing or dangling-symlinking.

**Steps to reproduce**

Install a local-path skill referenced by an agent, delete its source
directory contents so the stored SKILL.md copy cannot be recovered, and
start a run: before this change the skill vanishes from the runtime set
silently; on claude-local a pinned-but-unmaterializable version can fail
bundle preparation.

## What Changed

- `server/src/services/company-skills.ts` `resolveRuntimeSkillSource`:
both `.catch(() => null)` sites (version snapshot, runtime
materialization) now return the structured `{status: "missing", source,
detail}` shape the deliberate missing branch already used, with the
underlying error message in `detail`.
- `packages/adapter-utils/src/server-utils.ts`:
`isPaperclipSkillSourceMissing` is exported with a doc comment.
- `packages/adapters/claude-local/src/server/execute.ts`: missing
desired skills are filtered out of the prompt bundle and each one logs a
`[paperclip] Warning` with its detail to the run output.
- `cursor-local`, `gemini-local`, `kimi-local`, `opencode-local`,
`pi-local` `execute.ts`: mount loops (and the cursor/gemini injection
calls) skip missing entries instead of symlinking a nonexistent path.

## Verification

- `cd server && npx vitest run
src/__tests__/company-skills-service.test.ts` — new test pins the
missing-with-cause entry for a failed materialization. Nine pre-existing
project-workspace tests in this file fail on my machine at clean
`master` too (environment-specific); their count is unchanged by this
PR.

flagged missing). All shipped consumers either intersect with desired
sets, already handle `sourceStatus: "missing"`, or now skip missing
entries at mount time. The snapshot layer already understood the missing
shape via the `materializeMissing: false` path, so downstream contracts
are unchanged.

- Claude Fable 5 (`claude-fable-5`, Anthropic) with extended thinking

literal, percent-encoded text `{id}` — which 404s. The source is adapter
prompt text: the gemini adapter's API note embeds a curl example with
`/api/issues/{id}/checkout` in the URL, the shared agent prompt template
mentions `/api/issues/{issueId}` endpoints, and the harness checkout
notice names `/api/issues/{id}/checkout`. Models copy these strings into
real requests.

**Expected behavior**

URL paths in prompt text must carry environment variables or real ids,
never brace placeholders, in every string an agent might execute
verbatim. Where a placeholder is unavoidable, the prompt must state
explicitly that the literal text must never be sent.

**Steps to reproduce**

1. Give an agent the gemini adapter's API access note.
2. Watch it call `curl ...
"$PAPERCLIP_API_URL/api/issues/{id}/checkout"` as written.
3. The server logs `POST /api/issues/%7Bid%7D/checkout 404`.

## What Changed

- gemini-local's API note curl example now uses `$PAPERCLIP_TASK_ID` and
tells the agent to substitute a real issue id when that variable is
absent — the same convention as the acpx engine's API note.
- The shared agent prompt template (`server-utils.ts`) uses
`$PAPERCLIP_TASK_ID` in its interaction-creation and resume-endpoint

Add `buildDocumentReviewContext` beside the existing plan builder. It
collects open annotation threads for all non-plan issue documents,
applies the same thread, comment, and character budgets across
documents, and reports truncation. Include the result as a new
`documentReviewContext` field in agent wake payloads and in the issue
wake-context route. Keep the plan context on its legacy builder and
field so plan-only wakes stay byte-for-byte compatible. Render the new
context in the adapter wake-payload text, and surface annotation counts
and the annotation panel for documents in the properties pane's Plans
and Artifacts tabs.

**Reason and benefit**

The floating annotation popover and persistent highlight UI landed
earlier; this change completes the loop so agent feedback on any issue
document reaches the assigned agent, not only feedback on the plan.

`docs/deploy/environment-variables.md` ("preserves the value", run-level
var "inherits the server-level value")
- **Steps to reproduce:** (1) set `PAPERCLIP_AUTH_PUBLIC_BASE_URL` to an
address not reachable from inside the server container, (2) set

**Problem or motivation**

Agent-posted user interactions (plan confirmations, structured
questions, suggested-task proposals, checkbox prompts) are written in
each agent's default style. Operators who want fast, unambiguous
decisions have no way to ask agents to use a controlled language for
exactly those decision points.

**Proposed solution**

Add an experimental instance setting,
`enableSimplifiedEnglishInteractions` ("Simplified English
Interactions"). When it is on, the server sets
`simplifiedEnglishInteractions: true` in the heartbeat wake payload. The
shared wake-prompt renderer, used by every adapter, then emits a
directive: write all user-interaction content in ASD-STE100 Simplified
Technical English, state what information the user needs to decide, and
state what happens for each choice. The directive applies to interaction
content only. Thinking, comments, documents, and other responses keep
their usual style.

**Alternatives considered**

Per-agent instructions work today, but someone must maintain them on
every agent. An instance-level toggle applies uniformly and turns off in
one place. Server-side rewriting of interaction payloads was rejected:
post-hoc translation is lossy and cannot add the decision context that
only the agent has.

**Roadmap alignment**

Extends the experimental settings surface with another opt-in
agent-behavior refinement, consistent with existing prompt-side flags.

## What Changed

- Added `enableSimplifiedEnglishInteractions` to the experimental
instance-settings zod schema, mirror type, and feature catalog (default
off, tier preference) in `packages/shared`.
- Server `instance-settings.ts` normalizes the flag on both read

**What happened?**

The default local-agent prompt described `wake_assignee` as
acceptance-only for `request_confirmation`. This conflicts with the
server. The server wakes on every non-expired resolution. A recovery
follow-up test also used an already-normalized execution agent name key,
so it did not exercise the normalization seam.

**Expected behavior**

The prompt must state that `wake_assignee` resumes after acceptance or
rejection. It must direct acceptance-only flows to
`wake_assignee_on_accept`. The recovery regression must use a
display-style agent name key and prove that the follow-up path still
works after normalization.

**Steps to reproduce**

1. Read the default local-agent prompt in
`packages/adapter-utils/src/server-utils.ts`.
2. Compare its confirmation continuation text with
`queueResolvedInteractionContinuationWakeup` in
`server/src/routes/issues.ts`.
3. Observe that the prompt gives acceptance-only semantics to
`wake_assignee`.
4. Inspect the recovery hand-back test and observe that its execution
name key is already normalized.

Keep the remote path drop-only. That keeps the gap open. Move the change
into SSH too. That expands scope beyond the target transport and adds

provide structured action references. Agents could also set presentation
directly, but that would weaken the current board-only security

This change refines the completed “Self-healing runs & automatic
recovery” and “Enforced Outcomes” roadmap areas. It does not add a
competing roadmap capability.

**Additional context**

The scope covers shared comment validation, server recovery notices,
agent-comment derivation, and recovery prompt text. No database
migration is needed because presentation data already uses JSON.

perf(heartbeat): reuse task sessions for issue-scoped timer wakes and bound control-plane write retries (#10350)

per-heartbeat orchestration overhead: every timer wake discards the task
session (full session start + re-orientation), and in degraded
environments agents repeatedly retry the same failing control-plane
write.
- **Expected behavior:** Heartbeat orchestration should add minimal
wall-clock overhead on top of the underlying harness; issue-scoped
continuation work should not pay a fresh-session tax each interval.
- **Steps to reproduce:** Run a fixed benchmark task set once through
Paperclip issue heartbeats and once via direct harness invocation with
the same model/config; compare wall-clock totals.

- `server/src/services/heartbeat.ts`: `shouldResetTaskSessionForWake`
now resets only for `heartbeat_timer` wakes with no derivable task key

- Prompt/skill text changes alter agent guidance; the new rule is scoped
narrowly to repeated failures of the same control-plane write.
- No migrations, no API or schema changes, no dependency changes.

wake-payload description handling and found none besides the merged
#10151.

**Bug:** After #10151, adapters that inject the `Paperclip task context`
markdown (ACPX engine lanes, claude-local CLI, hermes server and
gateway) receive the issue description twice in a single prompt — once
in the wake prompt's `Issue description:` block and once in the task
markdown. Separately, resume deltas re-send the full description (up to
12k characters) on every wake even though the persistent session already
received it.

**Expected behavior:** The description appears exactly once per prompt
on every lane, and resume deltas only carry it when the resuming session
may not have seen the brief (assignment-shaped or recovery wakes),
leaving an explicit fetch breadcrumb otherwise.

**Reproduction:** Wake a claude-local or ACPX agent on an issue with a
description and inspect the assembled prompt: the description text
appears in both the wake-payload block and the task-context block. Wake
the same session again via a comment: the full description is present
again in the resume delta.

**Affected version:** Current `master` (with #10151 merged).

**Deployment mode:** Adapter-backed heartbeat execution, local and
sandboxed lanes.

## What Changed

- `renderPaperclipWakePrompt` accepts `suppressIssueDescription`; the
four task-markdown lanes pass it so the task markdown stays the single,
uncapped description carrier there.
- Non-assignment resume deltas omit the description and emit `- issue
description: omitted from this resume delta; fetch the issue if you need
the latest brief`. Assignment-shaped reasons (`issue_assigned`,
`issue_reopened_via_comment`, `issue_recovery_action_restored`,
`issue_tree_restored`) and recovery wakes still deliver the full brief.
- `buildPaperclipTaskMarkdown` gains `includeDescription`; the server
now also publishes `context.paperclipTaskMarkdownCompact` (description
stripped, directives and wake comment kept), and the new
`selectPaperclipTaskMarkdown` helper picks the right variant under the

`PAPERCLIPAI_CMD` could point at a missing checkout-local CLI module.

- **Reproduction:** create a plugin agent session for a local adapter,
call `sendMessage()` with a non-empty prompt, inspect the adapter prompt
and terminal session event.
- **Affected baseline:** `b517b887a` on `master`, local trusted
deployment with plugin host services and `claude_local`; `codex_local`
shared the wake-rendering gap because both use the common Paperclip wake
prompt renderer.

## What Changed

- Added a typed `agentMessage` wake payload rendered by the shared
adapter prompt path used by `claude_local`, `codex_local`, and other
local adapters.
- Labeled session content as user-supplied and explicitly
non-authoritative: it cannot expand authorization, permissions, task

run comments; no raw stdout or secrets are added.
- Consumers that ignore the new field remain compatible, and successful

evidence. This change instead introduces cause-specific routing and
recovery contracts across the recovery lifecycle.

### What happened?

When an issue became stranded, recovery generally selected an owner
through the same fallback ladder and rendered the normal execution
contract. That made the recovery wake look like ordinary deliverable
work, even when the correct action was to retry the original agent,
repair its runtime, or wait for a provider quota reset.

### Expected behavior

Recovery should select a response by failure cause, tell the recipient
to recover rather than complete the deliverable, suppress takeover wakes
for provider quota waits, and return repaired work to its original
assignee unless the recovery owner explicitly completes it.

### Actual behavior

Recovery could escalate transient failures to management, omit the
cause-specific next action from the wake, and leave the recovery owner
assigned after the runtime problem was resolved.

### Impact

The generic path creates avoidable management work, ownership churn, and
budget consumption while obscuring whether recovery successfully
returned work to the responsible agent.

## What Changed

- Added cause-keyed routing for process loss, missing disposition,
provider quota limits, Codex output inactivity, workspace validation
failures, and fallback recovery causes.
- Added recovery-scoped wake rendering that replaces the generic
execution contract with the failure summary, original assignee, attempt
count, next action, and cause-specific playbook instruction.
- Propagated the structured recovery contract through all built-in
adapter execution paths, including Hermes local and gateway adapters.
- Added provider-quota wait monitoring so capacity failures schedule the
original assignee instead of enqueueing a takeover wake.
- Added hand-back behavior and `handed_back` / `owner_completed` outcome
accounting when recovery actions are resolved.
- Added focused routing, renderer, quota-monitor, and hand-back
regression coverage plus implementation-spec documentation.

## Verification

- Adapter prompt plumbing changes across built-ins, covered by shared
renderer tests and compile-time call signatures.

fix(adapters): forward resolved adapter env to local agents, keep runtime env authoritative (#9617)

No public GitHub issue exists, so the underlying bug is described inline

**What happened**

Env keys configured on a local adapter (plain values and `secret_ref`
bindings, resolved server-side into plain strings) did not reliably
reach the spawned agent process. Two distinct gaps: (1) when merging
config env into the process env, a config key in the reserved
`PAPERCLIP_*` namespace could overwrite a Paperclip-managed runtime
variable (identity, wake, workspace, API access); (2) a warm-handle /
resumable ACP session computed its reuse fingerprint from
`secretManifestHash` only, which misses plain-value edits and
same-version secret rotations — so a resumed session kept serving stale
env and never re-launched with updated values.

**Expected behavior**

Non-`PAPERCLIP_*` adapter env (plain + resolved secret values) is
forwarded to the agent process; a change to any applied forwarded value
invalidates a warm/resumable session so the next launch sources the
latest env; configured `PAPERCLIP_*` entries can never override
Paperclip runtime env, while an explicitly configured
`PAPERCLIP_API_KEY` (stable per-run config) is still honored and its
rotation also busts the session.

**Steps to reproduce**

Configure an adapter with an env key (e.g. a `secret_ref` API key) and a
resumable ACP session. On resume, the updated env value is not sourced;
separately, a `PAPERCLIP_*` config key overrides the runtime value.

**Deployment mode**

Local adapters (Claude / Codex) via the shared adapter-utils execution
path.

## What Changed

- `packages/adapter-utils/src/server-utils.ts`: add
`isPaperclipRuntimeEnvKey` and, in
`refreshPaperclipWorkspaceEnvForExecution` (used by all local adapters),
skip a `PAPERCLIP_*` config key when Paperclip has already assigned it
this run; all other keys still forward.
- `packages/adapter-utils/src/acpx-engine/execute.ts`: apply the same
`PAPERCLIP_*` non-override rule (via the shared helper) when merging
config env, capture the applied config env in `resolvedAdapterEnv`, and
fold a stable `adapterEnvHash` of it into the session fingerprint so an
env change forces a fresh launch. Per-wake `PAPERCLIP_*` runtime vars
are assigned earlier and never enter that map, so they stay out of the
hash; stable configured `PAPERCLIP_*` values (e.g. an explicit
`PAPERCLIP_API_KEY`) are included so rotating one busts the session.
- Added unit/integration tests for plain + secret forwarding,
`PAPERCLIP_*` non-override, the explicit-API-key path, fingerprint
refresh-on-env-change vs. stable-across-wakes, and rotation of a
configured `PAPERCLIP_API_KEY`.

## Verification

- `npx vitest run packages/adapter-utils/src/acpx-engine/execute.test.ts
packages/adapter-utils/src/server-utils.test.ts` → 2 files, 111 tests
passing (includes the new cases).
- Tests assert: forwarded plain/secret values appear in the spawned
wrapper `.env`; a `PAPERCLIP_*` config key does not override the runtime
value; changing an applied forwarded env value (including a rotated
`PAPERCLIP_API_KEY`) changes `configFingerprint`, while a new wake with
the same config env keeps it stable.

## Risks

Low risk. Behavior change is limited to (a) config env no longer
overriding `PAPERCLIP_*` runtime vars — a security-positive tightening —
and (b) a resumable session re-launching when its applied config env
changes, which is the intended fix. Per-wake `PAPERCLIP_*` churn is
deliberately excluded from the fingerprint so normal sessions still
resume across heartbeats. Existing sessions get a new fingerprint once
on first deploy (the added `adapterEnvHash` field), which is expected.
No secret values are logged (key-name redaction in the adapter plus
manifest-driven redaction of `meta.env`).

## Model Used

Claude Opus 4.8 (Anthropic), model id `claude-opus-4-8`, extended
reasoning with tool use.

## What Changed

- Added a shared Bubblewrap process wrapper with opt-in
`filesystemScope: "workspace"`, managed/extra path mounts, private
`/tmp`, and Linux-only validation.
- Added `networkScope: "deny" | "allowlist"`; both use a private network
namespace, while allowlist mode exposes an exact-host HTTP(S) proxy over
a Unix-socket bridge.
- Wired Codex and Claude local CLI execution through the wrapper and
forced scoped auto runs onto the CLI lane because ACP processes are not
covered.

fix(adapters): inject execution contract once per fresh heartbeat (#9469)

full contract.
- Keep the contract in resume deltas, and add `includeExecutionContract`
for adapters that do not render the default heartbeat template.
- Opt `openclaw-gateway` into wake-payload contract rendering so
template-less gateway runs retain the guidance.
- Omit comment-batch acknowledgement/fetch guidance and empty `pending
comments` / `latest comment id` metadata when a fresh wake has no
pending comments.
- Add regression and acceptance coverage proving composed fresh prompts
contain `Execution contract` exactly once while resume and template-less
paths retain it.

Measured effect: the fresh zero-comment wake block drops from 1,840 to
855 characters (about 300 tokens saved per fresh heartbeat; about 220 on
comment wakes), and the composed fresh prompt contains `Execution
contract` once instead of twice.

### What happened?

When `@paperclipai/adapter-utils`'s `runChildProcess` reaches
`timeoutSec` and the spawned child ignores SIGTERM, the SIGKILL
escalation on the **direct-child fallback** path
(`signalRunningProcess`, taken on win32 or whenever `process.kill(-pgid,
…)` is unavailable or throws) never fires, so the child outlives its
deadline indefinitely. Root cause: the escalation is gated on
`!running.child.killed`, and `ChildProcess.killed` reflects only that a
signal was *successfully sent* (per the Node docs it "does not indicate
that the child process has been terminated"). After the deadline
SIGTERM, `child.killed` is already `true`, so `!child.killed` is `false`
and the follow-up SIGKILL is suppressed.

### Expected behavior

After the grace window, a child that is still alive is force-killed with
SIGKILL regardless of whether SIGTERM was already sent — the hard
timeout should guarantee termination (except true uninterruptible
D-state) on every platform/configuration.

### Steps to reproduce

1. Spawn a child that installs a no-op `SIGTERM` handler and never exits
(e.g. `process.on('SIGTERM', () => {}); setInterval(() => {}, 1000)`).
2. Drive it through the direct-child fallback, i.e.
`signalRunningProcess({ child, processGroupId: null }, …)` (the path
used on win32 / when group signaling is unavailable).
3. Send SIGTERM (the child swallows it; `child.killed` becomes `true`),
then send SIGKILL.
4. On the pre-fix `!child.killed` guard the SIGKILL call is a no-op and
the PID survives past its deadline. Covered by the new regression test
in this PR.

### Paperclip version or commit

Reproduces on `master` (the `signalRunningProcess` fallback). Also
present in published `@paperclipai/adapter-utils` (e.g. `2026.325.0`),
where the same `!child.killed` guard sits on the single direct-child
escalation path.

_Searched the open PR list for duplicates/related work on
`runChildProcess` / `signalRunningProcess` / SIGKILL escalation; found
none._

## What Changed

- `packages/adapter-utils/src/server-utils.ts`: in
`signalRunningProcess`, replace the direct-child fallback guard
`!running.child.killed` with `running.child.exitCode === null &&
running.child.signalCode === null` (real liveness). The process-group
path is unchanged.
- `packages/adapter-utils/src/server-utils.ts`: `export`

feat(prompt): one-time execution-workspace branch guard in wake prompt (PAP-13326) (#9319)
## Summary

When a task runs in a branch-pinned execution workspace, agents
sometimes switch or rename the workspace branch, which breaks the
worktree contract. This adds a short, one-time prompt hint telling the
agent to stay on the pinned branch.

- **heartbeat.ts**: after the execution workspace is resolved, attach
`executionWorkspace: { branchName }` to the wake payload (only when a
branch pin exists — agent-home runs without a branch are untouched).
- **server-utils.ts (adapter-utils)**: normalize the new payload field
and render one bullet in `renderPaperclipWakePrompt`:
> `- execution workspace branch: you are running in an execution
workspace on branch \`<name>\`. Do not switch, rename, or re-point this
branch; keep all commits on it.`
- The hint renders **only on non-resumed sessions** — resume-delta

## Tests

- `server-utils.test.ts`: branch guard renders on first prompt, absent
on resumed-session prompts, absent when no branch is pinned; payload
round-trips through `stringifyPaperclipWakePayload`.
- `heartbeat-workspace-branch-containment.test.ts`: the finalize-path
adapter mock now asserts the wake payload the adapter receives carries
the branch pin matching `context.paperclipWorkspace.branchName`
(end-to-end heartbeat wiring, embedded postgres). All 6 pass.
- Full `server-utils` (56) and acpx-engine execute (34) suites green;
adapter-utils typechecks clean; no new server tsc errors.

**Summary**

Remove the bundled `paperclip-dev` skill (now maintained in its own
external repo) and retire the `required` / `requiredReason` skill
attribute and the `paperclip_required` skill origin, which only existed
to support it.

**Problem or motivation**

`paperclip-dev` is the only bundled skill that was ever marked
"required". Now that it lives in a separate repository, shipping it
inside the core tree is wrong, and the entire `required` surface (a type
field, a validator field, a synthesized `paperclip_required` origin, UI
"Required by Paperclip" section, and required-skill merging in the
desired-skills calculation) becomes dead weight. The `required` value is
computed at request time and never persisted, so it can be removed
cleanly without a migration or compatibility shim.

**Proposed solution**

Delete `skills/paperclip-dev/`, drop the `required` / `requiredReason`
fields and `paperclip_required` origin everywhere they are produced or
consumed, collapse managed-skill origin to a single `company_managed`
value, and simplify `resolvePaperclipDesiredSkillNames` to return only
the configured desired set.

**Alternatives considered**

Keeping the `required` attribute as a no-op for forward compatibility —
rejected because it is request-time only (nothing persists it), so
leaving it in place is pure dead surface area with no callers.

**Roadmap alignment**

Internal cleanup / dead-code removal that simplifies the adapter-skill
surface; it does not introduce or duplicate any planned core feature in
ROADMAP.md.

## What Changed

- Deleted bundled `skills/paperclip-dev/` (moved to a separate repo).

- `server/src/services/company-skills.ts`: dropped the `required =
sourceKind === "paperclip_bundled"` synthesis when listing runtime skill
entries.
- `server/src/routes/agents.ts`: removed required-skill merging from the
desired-skills calculation in the persist-config path and the
unsupported-snapshot path (keeping the current version-aware
`desiredSkillEntries` structure).
- `ui/src/pages/AgentDetail.tsx`: dropped required-based filters, the
required tooltip, and the entire "Required by Paperclip" section from
the agent skills tab; storybook fixtures in
`ui/storybook/stories/acpx-local.stories.tsx` cleaned up to match.
- Tests: deleted the `required: false` case in
`paperclip-skill-utils.test.ts` and the "keeps required bundled skills
installed" case in every `*-local-skill-sync.test.ts`;
`acpx-local-execute.test.ts`, `cursor-local-execute.test.ts`,
`cursor-local-skill-sync.test.ts`, `agent-skills-routes.test.ts`, and
`packages/adapter-utils/src/server-utils.test.ts` were updated to drop
removed fields and map `origin: "paperclip_required"` →
`"company_managed"`.
- `server/src/adapters/registry.ts`: two `as unknown as
ServerAdapterModule["..."]` casts on `hermesListSkills` /
`hermesSyncSkills` (matching the existing `executeHermesLocal` pattern).
`hermes-paperclip-adapter@0.2.0` still depends on the published
`@paperclipai/adapter-utils` which keeps the retired
`paperclip_required` variant; the cast bridges the
workspace-vs-published type mismatch at the registry seam and can drop
once hermes upgrades.

- Affected vitest suites pass: `paperclip-skill-utils`, `server-utils`,
all eight `*-local-skill-sync`, `agent-skills-routes`, and the
`acpx`/`cursor`/`pi` execute suites.

## Risks

- Behavioral shift in the agent skills UI: the "Required by Paperclip"
section disappears. No bundled skill is required anymore, so this only
affects environments that previously surfaced `paperclip-dev` as a
forced-on row; those installs will see the skill move into the regular
"company-managed" list (and be uninstalled on next sync unless
explicitly listed as desired).
- Existing agents may still have the string `"paperclip-dev"` in their
persisted `desiredSkills`. That entry is inert (no source for it to
install from); a one-time DB cleanup is out of scope. Low risk.
- Hermes adapter type bridge: two casts in `registry.ts` paper over a
type-only divergence between the workspace `@paperclipai/adapter-utils`
and the published version still pinned by
`hermes-paperclip-adapter@0.2.0`. Runtime behavior is unaffected because
the retired `paperclip_required` value is no longer produced by anything
in this tree. The casts can be removed once hermes upgrades its
dependency.

approvals, or submit approval comments.

## Proposed solution

Add a task-watchdog data model, scheduler/classifier, scoped mutation
guard, adapter wake context, API/UI configuration surfaces, and
documentation so watchdog agents can review stopped task subtrees under
explicit boundaries.

## Alternatives considered

Reuse the existing recovery-action flow only. That would keep
stopped-work detection implicit, make per-task watchdog assignment
harder to expose in the UI, and would not provide a durable
scoped-review issue for stalled task trees.

## Roadmap alignment

This is Paperclip control-plane lifecycle infrastructure for task
execution and recovery. I checked `ROADMAP.md`; this PR does not
duplicate an existing planned core item.

## Risks

- Behavioural shift: SSH remote execution previously passed inherited
host env
  vars verbatim. Code that relied on that (e.g. a remote command somehow
  expecting the host's `PATH`) will see different behaviour. None of the
  adapter code in this repo has such a dependency.
- Edge case: if a caller explicitly sets `PATH` to the same value as the
host's
`PATH` (literally — same exact string), the sanitizer drops it as a
leak.
  In practice no caller constructs the env this way.
- Windows host: case-insensitive lookup handles `Path` vs `PATH`
correctly.
  Tested.

- Worktree path is now nulled out on remote (it has no meaning there).

  that previously read the value defensively will continue to work.

Fix CEO AGENT_HOME paths and centralize workspace env propagation (#4551)

strings" behavior.
