refactor: remove automatic productivity reviews (#13263)

## Linked Issues or Issue Description

Refs #5897. That request describes unwanted automatic productivity
reviews and asks to preserve existing tasks. This change retires the
feature instead of adding another configuration switch.

Related prior approaches: Refs #9191, Refs #12489. Those changes
excluded infrastructure failures or bounded review creation. This
removal replaces the detector rather than tuning its thresholds.

## What Changed

- Delete the scheduled detector, automatic task creation, evidence
refresh, and productivity continuation holds.
- Remove computed productivity fields, special attention items, badges,
and Storybook fixtures.
- Retain historical origin values, decision compatibility, and recovery
recursion exclusions. Add no migration and change no existing task data.
- Update the execution contract. Replace feature tests with regressions
for legacy task reads, ordinary attention, and bounded continuation in
the presence of an old review.

## Verification

- Targeted attention, issue-route, startup, and UI tests: 4 files and
101 tests passed.
- Updated issue-route and UI tests: 2 files and 61 tests passed.
- Bounded continuation regression: 2 cases passed, including a legacy
review plus pre-dispatch cancellation churn.
- `pnpm check:token-gates`: all four gates passed.
- `git diff --check`: passed.
- `pnpm build-storybook`: passed.
- Greptile: 5/5 on `a5a612eea`, with no actionable findings.
- Scheduler and historical recovery regressions: 2 files and 28 tests
passed.
- Repository `pnpm -r typecheck` and `pnpm build`: passed.
- The complete `pnpm test:run` suite passed across the CI server,
serialized-server, and workspace shards on `a5a612eea`. Stopped the
duplicate local monolithic run after the full CI suite passed; no
completed local full-suite result is claimed. The targeted local suites
above passed.
- CI serialized shard 5 initially hit a 10-second timeout in the first
interaction-route test. The complete file passed locally (78 tests),
then the single CI rerun passed.
- All CI gates are green, including the build and end-to-end suites.
- A local merge check against current `master` (`ce09ea40b`) completed
without conflicts.

## Risks

- API responses no longer include the computed `productivityReview`
field. Consumers must stop using it.
- The scheduler no longer creates management work from elapsed time, run
counts, or missing comments. This is the intended behavior change.
- Existing review tasks and explicit dependencies remain in place.
Historical origins still prevent recursive recovery treatment. No task
cleanup or data migration occurs.
- The native review handoff repair is separate from this removal.

## Model Used

OpenAI GPT-6 through Codex, with reasoning, tool use, and code
execution. The exact runtime model identifier and context-window size
are not exposed in this session.

fix: preserve recovery retries across restarts (#11817)

## Linked Issues or Issue Description

**What happened?**

A run that ended without a valid issue disposition could lose its repair
path after a server restart. Manager recovery could also change the
source owner. In addition, a parked or expired retry could make the
issue look healthy when no active work existed. Concurrent
reconciliation could also schedule the same repair attempt twice.

**Expected behavior**

Paperclip must keep bounded source and manager repair attempts across
restarts. Recovery ownership must stay separate from source issue
ownership. The server and user interface must report only a live retry
as active work. Each repair attempt must be scheduled at most once per
company.

**Steps to reproduce**

1. Start an agent run on an issue.
2. End the run without a valid issue disposition.
3. Let the first repair attempt schedule a retry.
4. Restart the server, let the retry time pass without a live run, or
start two reconciliation loops together.
5. Observe that the repair path can stop, the issue can show a false
healthy state, or duplicate retries can be created.

**Paperclip version or commit**

The problem existed on `master` before candidate head
`d8e620fe86bade7df18decac332007f5821ae04f`.

**Deployment mode**

The problem affects self-hosted servers and local builds that use
automatic recovery.

## What Changed

- Persist bounded source-owner and manager repair lineages with stable
fingerprints and retry limits.
- Resume incomplete disposition repairs after a server restart.
- Keep recovery ownership separate from source issue ownership and
enforce source mutation authority.
- Project live retry evidence into issue and blocker summaries.
- Show recovery owner, return owner, attempt count, and retry state in
the board user interface.
- Treat expired or parked retries as attention states unless a queued or
running attempt exists.
- Atomically deduplicate disposition-repair wake requests with a
company-scoped partial unique index.
- Reuse the winning run when concurrent reconciliation loses the
uniqueness race, without duplicate scheduling activity.
- Honor disabled on-demand wake policy before recovery scheduling and
again before delayed retry promotion.
- Keep the new index migration safe for lagging seeded databases that
already contain the index.
- Add server and user interface tests for recovery, restart, ownership,
retry, concurrency, and blocker states.
- Update the implementation and execution semantics documents.

## Verification

- Focused server recovery and ownership suites: 282 tests passed on the
repaired base candidate.
- Focused user interface recovery suites: 128 tests passed on the
repaired base candidate.
- Atomic-deduplication schema and recovery suites: 111 tests passed on
the first Greptile repair.
- Recovery and scheduled-retry wake-policy suites: 126 tests passed at
`d8e620fe86bade7df18decac332007f5821ae04f`.
- The exact lagging-source migration-order test passed after the index
migration became idempotent: 1 test passed and 62 unrelated tests were
skipped.
- `@paperclipai/db` and `@paperclipai/server` typechecks passed at the
current head.
- Migration generation and migration safety checks passed for migration
`0226_tan_colossus.sql`.
- `pnpm check:token-gates` passed on the repaired base candidate.
- `pnpm -r typecheck` passed on the repaired base candidate.
- `pnpm build` passed on the repaired base candidate.
- `pnpm test:run` passed 4,540 tests on the repaired base candidate.
Four fixed-port cases met listeners that already existed on the host.
- The two unchanged fixed-port files passed in an isolated network
namespace: 129 tests passed and 27 tests were skipped.
- Independent Security and QA reviews approved
`63c0423aab54c66f2293a20b0fb3f3b013ee3ba8`; exact-head re-review is
required after automated checks settle on
`d8e620fe86bade7df18decac332007f5821ae04f`.

## Risks

- Recovery orchestration affects issue liveness and ownership. The new
paths use bounded attempts, stable fingerprints, row locks, authority
checks, and database uniqueness.
- A conservative attention state can show more warnings when a scheduled
retry has no queued or running attempt. It does not hide stopped work.
- Migration `0226_tan_colossus.sql` creates a partial unique index on a
known-large table. Migrations run transactionally, so `CONCURRENTLY` is
unavailable. The matching disposition-repair key namespace is introduced
by this release, so deployed databases have no matching rows before the
index is added.

## Model Used

- OpenAI Codex from the GPT-5 model family used agentic reasoning, tool
use, and code execution. The runtime did not expose the exact model ID
or context window.
- Anthropic Claude Opus 5 used a 1M context window, tool use, and code
execution for part of the user interface repair, as recorded in the
commit history.

fix(ui): keep Archive available on every inbox item (#11636)

**What happened?**

Unread failed runs, approvals, and join requests showed a blue unread
marker but no Archive button. The user had to mark the item as read
before the old leading Archive control became available.

**Expected behavior**

Every Mine inbox item shows Archive on hover. Every item also keeps its
swipe-to-archive behavior.

**Steps to reproduce**

1. Open the Mine inbox.
2. Add an unread failed run, approval, or join request.
3. Hover the new item.
4. Observe that Archive is missing before this change.

**Paperclip version or commit**

Current `master` at `0e9b03832d`.

**Deployment mode**

Local development from source.

**Additional context**

PR #1860 improves the accessibility of the existing swipe gesture. PR
#9704 changes server-side archive resurfacing rules. Neither PR adds the
missing hover action to unread non-task rows.

## What Changed

- Reused the task-row Archive action for failed runs, approvals, and
join requests.
- Kept the unread marker visible in its own leading slot.
- Kept retry, approve, and reject actions beside the new trailing
Archive action.
- Added regression coverage for hover Archive and swipe wrappers on
every non-task Mine row type.
- Updated a test comment so the repository token gate does not treat
documented color literals as UI source.

## Verification

- `pnpm --filter @paperclipai/ui exec vitest run
src/pages/Inbox.test.tsx src/components/IssueRow.test.tsx
src/components/SwipeToArchive.test.tsx` — 46 tests passed.

- Greptile — 5/5 confidence with no open review threads.

## Risks

- Low risk. The change only affects Mine inbox row actions and
regression tests.
- The server archive and dismiss APIs do not change.
- Retry, approve, and reject behavior does not change.
- No documentation change is required because this fix restores the
expected inbox behavior.

fix(ui): keep mobile unread dots from indenting task rows (#10691)

**What happened?**

On mobile screens, an unread Inbox row placed the mark-as-read control
in the flex layout. The extra flex item moved the status and title to
the right. A first overlay position could also place part of the control
outside a row that clips overflow.

**Expected behavior**

Read and unread rows must use the same title and status positions. The
unread control must stay tappable inside the row. A parent-row chevron
must remain independently usable.

**Steps to reproduce**

1. Open the Inbox on a mobile viewport.
2. Compare read and unread rows with the same nesting depth.
3. Include an unread parent row with a collapse chevron.
4. Observe that the unread row content starts farther right than the
read row content.

**Paperclip version or commit**

The issue reproduced on the `master` parent of this pull request.

**Deployment mode**

Local dev (`pnpm dev`).

**Installation method**

Built from source.

## What Changed

- Reserve a mobile leading gutter whenever an Inbox row participates in
unread state.
- Position the mark-as-read control absolutely inside that gutter so it
does not add a flex column.
- Keep read and unread rows on the same mobile padding path.
- Keep the control inside overflow-clipping row containers and separate
from the parent-row chevron.
- Update the `IssueRow` regression test to verify absolute placement,
the internal gutter, and the absence of `order-first` layout.

## Verification

- `pnpm exec vitest run ui/src/components/IssueRow.test.tsx` — 16 tests
passed.
- `pnpm check:token-gates` — all token gates passed across 722 files.
- `pnpm -r typecheck` — passed.
- `pnpm build` — passed.
- `pnpm test:run` — the general server shard passed 3,448 tests and the
UI shard passed 3,362 tests. The CLI shard had one host-environment
failure because inherited temporary AWS access-key variables changed an
AWS doctor assertion from `pass` to `warn`.
- `env -u AWS_ACCESS_KEY_ID -u AWS_SECRET_ACCESS_KEY -u
AWS_SESSION_TOKEN pnpm exec vitest run
cli/src/__tests__/secrets.test.ts` — the affected file passed 8 tests in
an isolated environment.
- Mobile QA at 402 × 874 confirmed equal read/unread title positions, no
shift after marking a row as read, and an independently usable parent
chevron.

## Risks

- Low risk. The change affects only `IssueRow` mobile presentation and
its focused regression test.
- The main risk is breakpoint-specific placement. The component test
covers the responsive classes, and mobile QA covers the rendered
interaction.
- No API, schema, dependency, telemetry, or documentation contract
changes.

fix(ui): move IssueRow divider and hover wash to the row root (#10702)

**What happened**

- In the Inbox and Tasks list views, every row shows a 1px bottom
border, including the last row.
- Hovering a row dims/greys the row text instead of showing a background
tint behind the content.

**Expected behavior**

- List rows in Inbox and Tasks show no separator lines by default.
- Hover shows a subtle background tint behind the row content; the text
stays fully legible.
- The blocked inbox view keeps its intentional separators.

**Steps to reproduce**

1. Open the Inbox or Tasks list view.
2. Note the horizontal border under every row, including the last.
3. Hover a row and note the text greys out rather than the background
tinting.

**Root cause**

- PR #10526 restructured `IssueRow` from a single root `Link` into a
root `div` plus a full-row `absolute inset-0` overlay `Link` (to keep
header controls clickable). The divider and hover/selected/checklist
background classes moved onto the overlay `Link`. `last:border-b-0` no
longer matched (the Link is the first child of a multi-child div), and
the hover wash painted on top of the content instead of behind it.

**Paperclip version/commit**

- Base commit: `8b83d69e3` (branched from current `master`).

**Deployment mode**

- UI (web) list views: Inbox and Tasks.

## What Changed

- `ui/src/components/IssueRow.tsx`: moved the divider classes and the
hover/selected/checklist background wash from the overlay `Link` to the
root row `div`, so the tint paints behind the content and
`last:border-b-0` matches the real last row. The overlay `Link` now
keeps only `absolute inset-0` positioning and the focus ring. Renamed
the `hideDivider` prop to an opt-in `showDivider` (default `false`).
Kept `[&_button]:relative [&_button]:z-10` on the root so the Archive
button stays clickable above the overlay, and kept the `isArchiving`
collapse animation on the root row.
- `ui/src/components/IssuesList.tsx`: dropped the old `hideDivider`
usage (dividers are now opt-in).
- `ui/src/pages/Inbox.tsx`: dropped the old `hideDivider` usage.
- `ui/src/components/BlockedInboxView.tsx`: added `showDivider` so this
view keeps its separators.

## Verification

- `cd ui && npx tsc -b` — typecheck passes with the change.
- Manual (recommended for reviewer): in the Inbox and Tasks list views,
confirm no per-row bottom border and that the last row has none. Because
dark-mode `--border` is 10% white and near-invisible in screenshots,
assert the computed `border-bottom-width` on a row element rather than
eyeballing pixels.
- Hover a row: text stays legible; a background tint appears behind the
content.
- Inbox: the Archive button appears on hover and is clickable (the
overlay does not swallow the click).
- Blocked inbox view: separators still render.

## Risks

- Low risk. The change relocates existing Tailwind classes between two
elements of the same row and renames one internal prop; no data or API
surface changes. All `IssueRow` call sites were updated in this PR
(verified: no remaining `hideDivider` references).

## Model Used

- Claude, Opus 4.8 (`claude-opus-4-8`), extended thinking with tool use.

### Subsystem affected

`ui/` — React + Vite board UI.

### Problem or motivation

The compact status and priority icons can receive change handlers, but
their popover triggers are plain icon elements. They do not provide a
reliable click target, keyboard focus, or control label. Operators need
to change both values directly from the task header.

### Proposed solution

Use semantic button triggers in the shared status and priority
components. Keep the existing API update wiring and picker options. Keep
task-row navigation links separate from editable row controls. An
operator can select either icon, open its picker, and choose a new task
status or priority.

### Alternatives considered

A task-page-only wrapper would duplicate control behavior. Moving the
controls would also change the page layout. The shared components
already own the picker behavior, so a shared trigger fix is smaller and
more consistent.

### Roadmap alignment

This is a focused board UI usability and accessibility fix. It does not
duplicate a planned roadmap feature.

### Additional context

The task page already passes change handlers to these shared components.
This change makes that existing path interactive and accessible.

## What Changed

- Added semantic button triggers for compact and labeled status
controls.
- Added semantic button triggers for compact and labeled priority
controls.
- Added accessible current-state labels and keyboard focus styles.
- Separated issue-row navigation links from row controls to avoid nested
interactive elements.
- Added real popover interaction coverage and row semantics regressions.
- Added task-page regression tests for update requests.

## Verification

- Focused task-header and row suites passed with 131 tests.
- The final row, inbox, and picker regression suites passed with 83
tests.
- `pnpm --filter @paperclipai/ui exec tsc -b --force` passed.
- `pnpm run typecheck:build-gaps` reproduced the CI typecheck before the
fix. The forced UI build passed after the fix.
- `pnpm check:token-gates` passed.
- `pnpm -r typecheck` passed.
- `pnpm build` passed.
- `pnpm test:run` completed the server and UI partitions. One unrelated
CLI AWS doctor assertion saw injected static AWS credentials and
returned `warn` instead of `pass`. The exact test passed after those two
environment variables were removed.
- `git diff --check origin/master...HEAD` passed.
- All latest-head GitHub checks passed, including both e2e shards.
- Greptile passed at the required threshold with zero open review
threads.

## Risks

- The shared issue-row DOM now uses a full-area navigation link beside
native action buttons.
- Existing visual layout, pointer navigation, keyboard navigation, and
action behavior remain covered by row, inbox, and list tests.
- Read-only status and priority icon uses are unchanged.

In the Inbox, an unread row rendered its status icon and title farther

Read and unread Inbox rows keep identical status/title alignment while
preserving the unread affordance and any tree-expansion controls.
### Steps to reproduce

  1. Run Paperclip from `master` and open the Inbox at desktop width.
  2. Compare otherwise-equivalent read and unread leaf rows.
  3. Expand a task tree containing an unread depth-zero parent.
4. Observe the unread row indentation and the dot competing with the
parent chevron.
### Paperclip version or commit

Reproduced against the pre-fix `master` parent of this PR.

place the dot after the leading chevron.
- Reserve the same leading spacer for read and unread non-chevron Inbox
rows.
- Expand `IssueRow` and Inbox tests to cover leaf alignment, nested
rows, parent chevrons, fading state, and mobile behavior.

## Verification

- `pnpm exec vitest run ui/src/components/IssueRow.test.tsx
ui/src/pages/Inbox.test.tsx` — 2 files, 30 tests passed.

- The main visual risk is breakpoint-specific placement of the mark-read
dot; tests explicitly cover desktop absolute positioning and mobile

fix(ui): inbox/task list parity, nesting alignment, hover perf, and routine detail polish (#9317)
## Thinking Path

- **Problem**: after the design-system foundation (#9134) and component
convergence (#9240), the inbox/task lists still had interaction and
alignment gaps — hover lag on long lists, keyboard navigation that only
partly matched between the two lists, workspace/parent nesting whose
guides and chevrons didn't line up, an inbox that sat offset from the
task list, and a routine detail page with an odd double-scroll and a
bespoke sub-nav.
- **Proposed behavior**: the inbox and task lists share one interaction
contract (hover, keyboard nav, collapse, archive), list nesting aligns
to the status column with clean chevrons, list hover is CSS-only (no
per-hover re-render), and the routine detail page uses a fixed
header/sub-nav with a single scrolling content region and a sub-nav that
matches the primary nav.

## What Changed

- **List hover performance**: hover is now painted purely by CSS
`:hover` and records the hovered row in a ref, instead of writing
list-selection state on every `mouseenter` (which re-rendered 100–300
non-memoized rows per hover). Keyboard nav reads the ref so it still
continues from the hovered row; the keyboard band clears on the first
real mouse move so hover and keyboard selection never show two bands at
once. The row's `transition-colors` fade was removed so the highlight
snaps (no comet-tail). Measured on a 100-row scrub: worst frame **333 ms
→ 33 ms**, long frames (>50 ms) **12 → 0**, avg **41 → 60 fps**.
- **Inbox ↔ task-list parity**: keyboard navigation works on every inbox
tab (archive/read keys stay scoped to the archivable tab); group headers
and parent tasks collapse/expand with the arrow keys in both lists; the
task list gains the same j/k / arrows / Enter selection model as the
inbox; hover selection bands match; the `g` then `i` go-to-inbox chord
works app-wide.
- **List nesting & alignment**: the workspace group-header chevron lines
up exactly with the task chevrons below it (both lists); the
parent→child connector line drops from under the parent's **status
icon** rather than its chevron and breaks with a 14px gap around a
nested row's own chevron; and inbox rows line up with the task list
(read rows no longer reserve a mark-read column).
- **Inbox archive affordance**: moved from a bare `x` left of the status
icon to an `Archive` icon + label button on the right (before the
timestamp), revealed on row hover; the left slot now carries only the
unread dot; swipe-to-archive is unchanged.
- **Sidebar**: when no agent has a live run, the AGENTS section shows 3
recent agents (was 5) plus "See all agents"; the working-agents view is
unchanged.
- **Routine detail page**: the layout is bounded to the main scroll area
so the header (Run now + automation toggle) and the sub-nav stay fixed
and only the section content scrolls (was a page-level scroll competing
with a `sticky` sub-nav). The sub-nav items adopt the primary nav's
rhythm — row padding/height, inset rounded pill, type scale, and 16px
icons — and its background matches the main nav.
- **Task detail**: dropped the redundant `🔵` prefix the breadcrumb added
for live/in-progress tasks; the status glyph already conveys that state.

## Verification

- `pnpm check:token-gates` → 3/3 CLEAN
- `pnpm typecheck` → green (all packages)
- `cd ui && npx vitest run` → 2255/2255 (assertions updated in lockstep
where behavior changed)
- `pnpm --filter @paperclipai/ui build` → exit 0
- Storybook visual regression: run locally throughout (the
baseline-manifest archive is still unpublished, so CI cannot run this
suite — pre-existing condition from #9134). Every visible delta was
reviewed against a live instance and, where it was intentional (nesting
alignment, routine sub-nav restyle, unread-row shift), the affected
snapshots were re-baselined locally.
- Manual/measured: inbox + task lists (grouped and nested, light +
dark), hover-scrub frame timing, keyboard navigation, and the routine
detail scroll/alignment were exercised on a running instance.

## Risks

- Behavior-and-alignment changes concentrated in `IssueRow` /
`IssuesList` / `Inbox` (the shared task-row surfaces). The riskiest area
— the hover/keyboard-selection model — is covered by unit tests (updated
in lockstep) and was measured and driven live.
- The routine detail scroll change restructures that page's layout
container; verified the page's own scroll stays fixed while only the
section content scrolls.
- The visual suite cannot yet run in CI (unpublished baseline archive —
pre-existing); snapshot coverage is local-only until that lands.

## Model Used

- Claude Fable 5 (`claude-fable-5`, Anthropic) via Claude Code — agentic
coding with tool use (file editing, test execution, Playwright
measurement/screenshot verification); extended thinking enabled.

identity during issue creation, and heartbeat refuses malformed
git-sensitive workspace launches with one clear recovery action.
- Actual behavior before this PR: the run could reach adapter bootstrap
with an incoherent workspace context and fail with git errors such as
`fatal: not a git repository (or any parent up to mount point /srv)`.
- Root cause: child/follow-up issue inheritance preserved workspace
execution context without coherent project context. That let heartbeat
workspace resolution/adapter launch reach a fallback cwd instead of
refusing the malformed workspace state up front.

Add planning mode for issue work (#5353)

Darren Davison 12011fa9de feat: show sub-task count in title when parent is collapsed
When a parent issue is collapsed, its title is suffixed with
"(N sub-tasks)" so the count remains visible at a glance.
The suffix disappears when the parent is expanded.

dotta 49c7fb7fbd Unify unread badge and archive X into single column on Mine tab
The unread dot and dismiss X now share the same rightmost column on
the Mine tab.  When an issue is unread the blue dot shows first;
clicking it marks the issue as read and reveals the X on hover for
archiving.  Read/unread state stays in sync across all inbox tabs.
Desktop dismiss animation polished with scale + slide.
