**Subsystem affected**
Connected Apps, connection grants, secret resolution, native Git runtime
setup, webhook processing, and the Apps UI.
**Problem or motivation**
Users need to connect GitHub once and let agents use the correct GitHub
identity. A run should use a dedicated agent account when one exists.
Otherwise, it should use the responsible person's account. The
connection must survive token expiry, repository access changes, and
temporary instance downtime.
**Proposed solution**
Add user-owned and agent-owned GitHub grants to the existing connection
model. Resolve one identity for MCP, Git, `gh`, health checks, and
webhook bindings. Store provider tokens in the existing encrypted secret
system. Refresh expiring token pairs under the existing lease and
compare-and-swap path. Register signed Cloud webhook bindings and
process normalized pull request and installation events through a
durable local inbox.
**Alternatives considered**
An organization-wide GitHub token would lose person and agent
attribution. Environment variables alone would bypass the managed
connection and grant model. A new GitHub-only credential store would
duplicate the existing secret and access systems. GitHub App
installation tokens and private-key custody remain outside this first
version.
**Roadmap alignment**
This change implements the Connected Apps direction. It also extends the
shipped MCP Tool Gateway, per-agent secret access, and
action-attribution systems. It does not add a repository catalog. The
open repository catalog work in
[#11234](https://github.com/paperclipai/paperclip/pull/11234) is related
and complementary.

attribution, signed merged-PR webhook acceptance, durable
Cloud-to-instance delivery, and installation-access event processing.
Temporary branches and temporary repository access were removed
afterward.
- `pnpm check:token-gates` passed.
- `pnpm -r typecheck` passed before and after the rebase onto
`origin/master`.
- `pnpm build` passed.
- The focused connector suite passed 285 tests after the rebase.
- The full stable suite passed 5,790 tests and failed 22 tests across 8
general server files. The failures reproduced as shared-runner
environment issues. They included `/tmp` versus `/private/tmp`, closed
database connections, and invalid high ephemeral ports. The focused
connection tests pass in isolation.
## Risks
- Migrations add agent grant subjects and a durable connection-event
inbox. Migration numbering and safety checks pass.
- A raw GitHub user token enters the agent process for Git and `gh`.
Per-tool Ask-first controls cannot limit those shell operations. The UI
warns users about this boundary.
- GitHub App user tokens can be non-expiring. Paperclip performs a
continuity check every 30 days, but provider revocation still requires a
reconnect.
- The webhook path accepts only signed and bounded payloads. It stores a
minimal normalized record and no raw provider payload.
- GitHub repository permissions remain authoritative. Removed access can
make a cached repository count temporarily stale, but runtime access
fails immediately.

**What happened?**
The server could record hot-restart intent, but startup did not rebuild
native runner ownership. A live runner could not re-register its PRP
authority. A dead runner could not resume the exact native and provider
session on the same heartbeat run. Generic recovery could then block the
issue or create replacement work.
**Expected behavior**
A live native runner must reconnect with the same PID and logical
identities. A dead runner must resume the same durable session and
heartbeat run with only a new operating-system PID. A proposed or
terminal result must finalize once before any provider turn starts.
Ambiguous process or session evidence must stay blocked without a signal
or duplicate spawn.
**Steps to reproduce**
1. Start a Paperclip Runner heartbeat and wait for an active provider
turn.
2. Restart only the Paperclip server, with or without a hot-restart
marker.
3. Observe that the old startup path does not reconstruct the native
control-plane authority.
4. Kill both the server and runner after a provider checkpoint.
5. Observe that the old path cannot resume the exact native session on
the original heartbeat run.
**Paperclip version or commit**
The defect was reproduced from commit

prefix instead of the company that owns the workspace.

Remove the old task recovery experiment and its unused API surface. Make
task watchdog controls available without the removed flag. Put worktree
execution and managed environment controls in the developer section.
Resolve direct workspace links from the workspace owner and reject a
company prefix that does not own the workspace.

The smaller settings page is easier to understand. The server keeps only
the dependency wake backstop that it still uses. Workspace links open

Refs #12202. This pull request replaces the question-delivery foundation
from that stale task-thread pull request. The task-thread projection
will follow in a smaller pull request.
**What happened?**
Question answers were stored in the issue interaction. The server then
made one in-memory continuation wake. A server stop between those
operations could leave the answer stored but not delivered. The combined
native task-thread pull request also made this behavior hard to review
separately from UI changes.
**Expected behavior**
The answer and its delivery receipt must commit in one transaction. The
server must retry pending receipts after a restart. Existing direct
adapters must keep the current wake path. A native runtime may use the
optional steering seam, but this pull request does not enable native
steering in production.
**Steps to reproduce**
1. Create an `ask_user_questions` interaction.
2. Answer the interaction.
3. Stop the server before the continuation wake completes.
4. Start the server again.
5. On current master, no durable record tells the server to retry the
answer delivery.
**Paperclip version or commit**
Current `master` at `4d82f5eae`.
## What Changed
- Add the `issue_question_response_deliveries` table and migration.
- Store only routing state, a correlation ID, and a payload digest in
the delivery row. The answer remains in the existing interaction result.
- Commit an answered interaction and its pending delivery row in one
transaction.
- Add bounded claims, retry recovery, cumulative terminal state, and
content-free activity records.
- Keep every built-in direct adapter and external adapter on the
existing heartbeat wake path.
- Add an optional native steering seam. No production caller supplies
that seam in this pull request.
- Retain the provider-neutral `paperclip.question_set.v1` presentation
on recovered interactions.
- Run delivery immediately after an answer and sweep pending rows at
startup and on the existing server interval.
- Add focused database, service, route, startup, adapter-matrix, digest,
and duplicate-delivery tests.

- Secret patterns were not present in the changed text files.
- The repository token gates currently report violations from unchanged
files on `master`. This pull request does not change those files.
## Risks
The main risk is routing a direct-adapter answer into a native session.
The service checks the persisted runtime mode, and the adapter matrix
proves that all direct adapters use only the existing wake path. The new
table is additive. It has foreign keys, unique correlation constraints,
bounded attempts, and status checks. Activity records omit question and
answer content.

OpenAI Codex, GPT-5 family. The client does not expose the exact
deployment ID or context window. Agentic reasoning, tool use, and code
execution were enabled.

**Problem or motivation**
The server cannot authenticate runnerd, commit PRP events before ACK,
authorize semantic tools, or enter native finalization from a durable
runner result. The application must have this hidden boundary before a
guarded adapter can use the runner.
**Proposed solution**
Add an authenticated PRP WebSocket authority and register it only for
one exact persisted native Codex run. Bind each connection and event to
the company, issue, agent, run, runner, session, turn, item, and
verified runner identity. Commit each event before its cumulative ACK.
Project only authorized same-task read tools. Rebuild the accepted
result and finalization record from durable result and terminal events.
**Alternatives considered**
The server could expose a broad runner API key or route semantic calls
through existing adapter endpoints. Those options grant too much
authority and weaken replay recovery. The server could also add the
user-facing adapter in this pull request. That option would mix rollout
selection with the transport trust boundary and make legacy
compatibility harder to review.
**Roadmap alignment**
This work supports the shipped enforced-outcomes, governed-tool, and
self-healing-run milestones. It does not add a new roadmap surface.
## What Changed
- Add the durable PRP server authority with one-use bootstrap tickets,
reconnect leases, encrypted frames, bounded state, cumulative ACKs, and
idempotent commands.
- Add `/api/runner/v1/connect/:runId`. Derive its `ws://` or `wss://`

## Risks
- The server installs the WebSocket route at startup. An unregistered or
malformed run path fails closed and creates no native record.
- Bootstrap tickets are one use. The private state directory uses mode
`0700`, and the state file uses mode `0600`. The file stores derived
authentication verifiers and never stores raw tickets or lease tokens.
- The journal has explicit frame, command, event-window, and file-size
bounds. A bound violation closes the runner connection or rejects the
command.
- A runner event reaches the database before its ACK. A crash between
event commit and ACK causes a byte-equivalent replay, not a second
logical effect.
- The coordinator accepts only an existing queued or running native
Codex row with exact company, task, agent, runner, session, and
completion-contract ownership.
- Existing direct adapters do not call this service. They keep their
current execution, transcript, result, and finalization paths.
- The server has no production dependency on the private runner package.
Its build copies the compiled runtime into `server/dist`; the workspace
link is development-only. This adds no external package and does not
change the lockfile.

**Summary:** at server startup, `rewriteLocalUrlPort` corrupts an
explicit external `auth.publicBaseUrl`, leaking a dead
`PAPERCLIP_API_URL` to spawned agents.
**Steps to reproduce:**
1. Configure `auth.publicBaseUrl = https://<host>:8443` (an external
listener on a non-default port, e.g. Tailscale Serve).
2. Start the server (internal listen port `3101`).
3. Inspect a spawned agent run's env:
`PAPERCLIP_API_URL=https://<host>:3101`.
**Expected:** the agent-facing URL points at a reachable origin.
**Actual:** `curl "$PAPERCLIP_API_URL/..."` → `http_code=000` (HTTPS
against the plaintext HTTP port; TLS handshake fails). The fleet stays
healthy only because the runtime falls through its candidate list, but
any agent following the documented curl pattern silently hits a dead
endpoint first.

- New `server/src/url-utils.ts` with two intent-revealing helpers
(single source of truth):
- `rewriteUrlPort` — rewrite any explicit-port URL to a new port.
- `rewriteLoopbackUrlPort` — rewrite **only** loopback hosts; explicit
external URLs survive untouched.
- `isLoopbackHost` — bracket-tolerant so a URL hostname form `[::1]`
matches.
- `server/src/index.ts` (startup, the bug): `authPublicBaseUrl` now uses

Follow-on to #9921, which established run-bound agent secret access.
**Problem / motivation:**
Agents can receive credentials during work. There is no governed way for
them to propose a credential or binding without exposing plaintext in
work artifacts or immediately creating live access.
**Proposed solution:**
Store agent-authored proposals outside live secret tables. Encrypt each
proposed value and register exact-value redaction when Paperclip
receives it. Require an authorized human to approve or reject each
proposal. Approval executes through the normal write paths as the human
approver. Binding proposals can target only the proposer or its downward
reporting chain under the restrictive V1 policy.
**Alternatives considered:**
We rejected live secrets with a `proposed` status. That design would put
untrusted rows in resolver, list, and sync paths. It would also allow
uniqueness squatting. We rejected direct agent binding writes because a
binding is an agent-config write and must keep the existing human
permission gate.
**Roadmap alignment:**
This change extends the run-bound agent secret-access foundation in
#9921 with a governed proposal workflow.
## Security Verdict
Q0 SecEng verdict: **PASS-with-required-changes**. The review accepted
the separate proposal-table design and required the implementation to:
- fail closed unless both encryption and exact-value run redaction
registration succeed;
- scrub ciphertext idempotently on reject, withdraw, and expiry, with
audit-visible state;
- treat agent justification as hostile input and foreground action,
target, provenance, and approver permissions;
- snapshot and re-check the target agent plus reports-to chain at
approval to prevent org-chart laundering;
- make cascade approval atomic and fail closed if either secret creation
or binding authorization fails;
- deny low-trust, `skill_test`, `task_bridge`, and non-run-bound sources
consistently; and
- execute approval through the normal human secret/config write paths,
including protected-change gates.
Those requirements are implemented and covered by focused service,
route, and UI tests. Residual V1 risk remains the accepted 14-day
encrypted retention window. Proposal-time redaction also cannot clean a
value that leaked before the propose call.
## What Changed
- Added `company_secret_proposals`, migration `0207`, shared proposal
contracts, and a state-machine service for create, approve, reject,
withdraw, cascade, expiry, and ciphertext scrubbing.
- Added run-bound agent proposal routes and board review routes. The
routes derive provenance from authentication and enforce source
restrictions, company isolation, chain-of-command checks,
approval-as-approver, wake-on-resolution, and dual audit trails.
- Added durable per-run exact-value redaction registration so proposal
values remain redacted on later read surfaces.
- Added the Secrets **Proposals** tab and agent configuration **Proposed
access** rows. The UI shows fingerprint and length only. It also frames
agent justification as untrusted input, runs permission preflight,
supports approve and reject actions, and confirms cascades.
- Updated OpenAPI, agent skill guidance, API reference documentation,
and focused server and UI regression coverage.
- Rebased the branch onto current `master` and renumbered the proposal
migration after `0206`.
## QA Acceptance Results
Q5 QA verdict: **PASS — 9/9 acceptance criteria met**, with one Minor
non-blocking follow-up.
- **AC1:** proposed values never echo, never appear in live
lists/resolvers, and expose only fingerprint + length to board
reviewers.
- **AC2:** restrictive `self_and_reports` matrix passes: self/downward
allowed; upward/lateral denied.
- **AC3:** secret approval uses the normal create path, honors rename
overrides, records proposer/approver provenance, and scrubs ciphertext.
- **AC4:** approved bindings materialize and resolve through the target
agent's runtime list/fetch routes.
- **AC5:** pending-secret bindings require cascade; cascade succeeds
atomically and permission failures leave nothing applied.
- **AC6:** reject, withdraw, dependent rejection, and expiry paths scrub
ciphertext and preserve reasons/audit state.

The decisions desk and queue currently use different triage rules. They
can show stale repeated confirmations and can rank blocked work by an

The desk uses a can-wait group that has no clear arrival meaning. The
queue has fewer controls than the desk. Repeated pending confirmations

Group desk items by arrival date, and reserve Decide now for explicit
due dates. Use one toolbar and shelf model on both pages. Supersede

- The generated key uses an atomic hard link in the instance secrets
directory. An unsupported file system fails startup instead of replacing

Agents need a governed way to propose consequential work without
immediately mutating issues, especially when one choice can affect
several issue trees. Existing approvals and issue-thread interactions do
not provide a standalone object with typed options, target snapshots,
effect-level authorization, expiration, execution outcomes, and reusable
attention-feed presentation.
### Proposed solution
Add first-class Decisions that store options and typed inputs, surface
open proposals in the operator attention feed, validate target freshness
and the origin-agent/operator authorization intersection at decision
time, execute a bounded set of auditable effects, and retain terminal
outcomes. Decisions v1 supports comments, status and assignee changes,
follow-up issue creation, blocker resolution, and issue-tree
cancellation, plus bundle grouping, expiration/dismissal, rule-key
telemetry, and agent-facing API guidance.
### Alternatives considered
- Extend approvals with arbitrary effects: rejected because approvals
represent governed yes/no actions and would become an unsafe generic
mutation envelope.

**Problem (feature-request shape):** on a managed instance the control
plane can auto-install a bundled sandbox provider plugin via
`PAPERCLIP_MANAGED_CONFIG.plugins.autoInstall`, but cannot create the
environment row that makes the provider schedulable. The only boot-time
environment provisioning is Kubernetes-specific
(`PAPERCLIP_EXECUTION_MODE=kubernetes` + `PAPERCLIP_K8S_*`). A generic,
config-driven path is needed so any bundled provider can be provisioned
without per-plugin code or manual API calls.
## What Changed
- `server/src/services/managed-config.ts`: optional `environments`
top-level section — `[{ name, description?, provider, config? }]` —
validated fail-closed: unknown keys, more than one entry (the DB permits
exactly one Paperclip-managed sandbox row,
`environments_managed_sandbox_idx`), a `provider` not present in
`plugins.autoInstall`, `config.provider`, or secret-looking config keys
at any depth (`api_key`/`token`/`secret`/`password`/`credential`) all
refuse startup. Absent section ⇒ `environments: []`, so pre-section
documents keep booting newer builds.
- `server/src/services/environments.ts`: new provider-agnostic
`ensureManagedSandboxEnvironment({ name, description?, provider,
config?, extraMetadata? })` — idempotently owns the single managed
sandbox row: refreshes name/description/config each call, adopts the
slot across provider switches (dropping the stale
`managedKubernetesSandbox` marker), adopts a same-name unmanaged sandbox
row (stamping it managed) instead of colliding on
`environments_name_idx` every boot, and falls back to keeping the
current name if the desired name belongs to a different row.
`ensureKubernetesEnvironment` is now a thin wrapper that pins `provider:
"kubernetes"` and stamps the legacy marker.

Local Paperclip agents were sometimes inheriting
`PAPERCLIP_API_URL=http://192.168.1.50:3100` even when the server was

workspace. This keeps the runtime API on the loopback bind host for
local startup while still preserving LAN candidates for other callers.
## Problem
`choosePrimaryRuntimeApiUrl` preferred the first allowed hostname over
the actual loopback bind host. In the failing setup from NEE-327, that
exported `http://192.168.1.50:3100` into agent env even though
`http://127.0.0.1:3100` was the reachable control-plane URL.
## Solution
The primary runtime URL chooser now returns the normalized loopback bind
host before considering `allowedHostnames`. I added a focused unit test
for the chooser and a startup regression that verifies
`PAPERCLIP_RUNTIME_API_URL` / `PAPERCLIP_API_URL` stay on `127.0.0.1`
while the candidate list still includes the LAN hostname.

## Thinking Path

`config.host`)

> - Because the env var was unconditionally set after loading the
ConfigMap value, the ConfigMap-provided URL was ignored and replaced
with the internal localhost address

## What Changed
- `server/src/index.ts`: Wrapped `PAPERCLIP_API_URL` assignment in `if
(!process.env.PAPERCLIP_API_URL)` guard so externally provided values
are preserved
- `server/src/__tests__/server-startup-feedback-export.test.ts`: Added
tests verifying external `PAPERCLIP_API_URL` is respected and fallback
behavior is correct
- `docs/deploy/environment-variables.md`: Updated `PAPERCLIP_API_URL`
description to clarify it can be externally provided and the load
balancer/reverse proxy use case
## Verification
- Run the existing test suite: `pnpm test:run
server/src/__tests__/server-startup-feedback-export.test.ts` — all 3
tests pass
- Manual verification: Set `PAPERCLIP_API_URL` to a custom value before
starting the server and confirm it is not overwritten
## Risks
- Low risk — purely additive conditional check; existing behavior for
unset env var is unchanged
