**What happened?**
A second native heartbeat could reuse tickets, leases, command receipts,
sequence state, and run identity from the first heartbeat. Provider
phase and item identity could be lost before the UI read them. Redaction
could corrupt protocol discriminators while still missing malformed
credential tails. The task thread could fold progress into the final
response, hide failures, or show more than one final answer. Native
Codex also exposed approval modes that do not yet have a durable
approval bridge.
**Expected behavior**
Each heartbeat uses a new PRP authority epoch. Codex and OpenCode
preserve exact qualified provider sessions; ACPX emits an explicit
continuity event when its qualified process-replacement policy is used.
Every accepted provider event is presented, classified as internal, or
surfaced as unsupported. The task page shows chronological progress,
reasoning summaries, activity, Plans, interactions, terminal failures,
and exactly one final reply. Direct adapters retain their existing path.
**Steps to reproduce**
1. Enable the unified experimental Paperclip Runner setting.
2. Create a local native Codex, OpenCode, ACPX Claude, or ACPX Codex
agent.
3. Run response, Plan, structured-question/resume, restart,
cancellation, and failure scenarios.
4. Reload the task while active, waiting, failed, and settled.
5. On the old implementation, observe stale run authority, missing
classifications, incomplete output, or duplicated/folded replies.

- Rotates PRP control-plane, outbox, ticket, lease, command, receipt,
and sequence authority for each heartbeat while carrying forward only a
validated provider-session identity.
- Reads `control-plane-state.json`, validates both durable schemas and
lifecycle values, resumes coherent current runs, archives qualified
settled authority, and quarantines malformed or mismatched scoped state
without moving ambiguous live legacy state.
- Preserves Codex provider phase and stable item identities so
commentary remains progress and only `final_answer` becomes final.
- Adds raw OpenCode HTTP/SSE boundary coverage and canonical reasoning
lifecycle mapping.
- Makes ACPX normalization lossless for visible reasoning, tool
lifecycle metadata, stable bounded identities, Plan revisions,
structured requests, failures, and qualified process replacement. Only
the compatible terminal assistant message is promoted as final.
- Applies schema-aware redaction before generic JWT-shaped detection and
scans every diagnostic string leaf. Malformed raw/escaped quoted
credential tails are redacted in both server and durable Rust state.
- Restores snapshot-style chronological task presentation, expandable
tool activity, inline Plan cards, visible waiting/resume/cancel/failure
states, and exactly one final answer.
- Makes `never` the only qualified native Codex permission mode and
rejects unsupported persisted native modes with remediation. OpenCode
and ACPX policies remain intact.
- Keeps the unified experimental Runner setting as the only enablement
flag. Onboarding and direct Codex, Claude, and OpenCode stay on their
legacy execution/finalization paths.
- Adds cross-language goldens, authority/recovery/fault coverage, exact
response/count assertions, and native plus legacy acceptance scenarios.

## Risks

- Provider event formats can change. Closed validators and boundary

Paperclip exposes cheap-model profiles in adapter metadata, agent

board UI. Recovery work can select a different model profile from the
agent's configured model.
**Proposed behavior**
Paperclip uses the agent's configured model for normal work and recovery
work. Status-only recovery stays limited to coordination work. The API
rejects legacy model-profile configuration. A migration removes stored
model-profile values from existing agent, issue, and historical revision
records.
**Reason and benefit**
One model path reduces configuration, API, UI, and recovery complexity.
It also prevents status recovery from becoming a separate product-level
model-routing feature.
**Breaking changes**
This change removes model-profile fields and adapter capability
metadata. Existing stored model-profile values are removed by an
idempotent migration. The validators reject new legacy profile values
with clear errors.

`gpt-5.4-mini` is absent from the `models` list of both the
`codex_local` and `opencode_local` adapters, and `openai/gpt-5.5` is
absent from `opencode_local`. (`codex_local` already ships `gpt-5.5` —
it is the default model on master.) Operators who want these models must
type a manual model ID.
For `codex_local` that has a real side effect.
`isCodexLocalFastModeSupported` treats any *unknown* model as
Fast-mode-capable and passes `service_tier="fast"` and
`features.fast_mode=true` through to the CLI. Because `gpt-5.4-mini`
does not support Codex Fast mode, an agent configured with a manual
`gpt-5.4-mini` model ID and `fastMode` enabled silently sends overrides
the CLI cannot honor.
### Proposed solution
Add the three missing entries to the two `models` lists, and leave
`CODEX_LOCAL_FAST_MODE_SUPPORTED_MODELS` untouched. Listing
`gpt-5.4-mini` in `models` is precisely what makes it a *known* model,
so `isCodexLocalFastModeSupported` returns `false`, `buildCodexExecArgs`
omits the Fast mode overrides, and `fastModeIgnoredReason` is surfaced
to the operator.
### Alternatives considered
Adding `gpt-5.4-mini` to `CODEX_LOCAL_FAST_MODE_SUPPORTED_MODELS` as
well — rejected, because the model does not support Fast mode and the
overrides would be rejected at run time. Leaving the models unlisted so
operators keep using manual IDs — rejected, because that is the path
that silently enables Fast mode for a model that cannot use it.
### Roadmap alignment
Not core roadmap work. `ROADMAP.md` does not plan adapter model-list
maintenance; this is routine upkeep as upstream CLIs ship new models.

Creating, hiring, or updating a `codex_local` agent should either
persist isolated per-agent configuration or reject unsafe shared Codex
home configuration with a clear 422 response. The guard must not print
secret values.
**Steps to reproduce**
1. Create or update a `codex_local` agent without an explicit
`adapterConfig.env.OPENAI_API_KEY` override.
2. Run it on a host where the Paperclip server process has
`OPENAI_API_KEY` set.
3. Observe that the adapter process can inherit the host key unless
Paperclip persists a blocking empty override.
4. Set `adapterConfig.env.CODEX_HOME` to a shared path such as
`~/.codex` or the company-level `codex-home`.
5. Observe that the old code allowed the shared auth home instead of
returning a validation error.

Paperclip-known model. The selectable catalog still includes `gpt-5.5`,
`gpt-5.4`, `gpt-5.3-codex`, etc. for operators who want to pin

- Wire each of the 6 affected adapter `testEnvironment()`s to call
`maybeRunSandboxInstallCommand` before
`ensureAdapterExecutionTargetCommandResolvable`
- Pass `installCommand: SANDBOX_INSTALL_COMMAND` through
`prepareAdapterExecutionTargetRuntime` in each adapter's `execute()`
- Per-adapter install commands use npm globals where possible so
binaries land on a PATH segment the template already exports:
- claude → `npm install -g @anthropic-ai/claude-code`
- codex → `npm install -g @openai/codex`
- cursor → `curl https://cursor.com/install -fsS | bash`
- gemini → `npm install -g @google/gemini-cli`
- opencode → `npm install -g opencode-ai`
- pi → `npm install -g @mariozechner/pi-coding-agent`
SSH and local targets ignore `installCommand` (SSH runtime takes no such
param; local short-circuits before runtime prep), so this is a no-op for
non-sandbox environments.

install still lets the report run resolvability and hello probes. If a

The previous documentation parenthetical "(defaulting to ~/.codex/skills/)"
was misleading because Paperclip almost always sets CODEX_HOME to a
per-company managed home. Update index.ts docs, skills.ts detail string,
and execute.ts inline comment to make the runtime path unambiguous.

The Codex adapter was the only one injecting skills into
<cwd>/.agents/skills/, polluting the project's git repo. All other
adapters (Gemini, Cursor, etc.) use a home-based directory. This
changes the Codex adapter to inject into ~/.codex/skills/ (resolved
via resolveSharedCodexHomeDir) to match the established pattern.

Heartbeat service resolves cwd from task session, project primary
workspace, or agent home directory (~/.paperclip/instances/.../workspaces/).
Adapters receive workspace context and forward it as env vars and
session params. cwd is now optional in adapter config.

Codex adapter now auto-injects Paperclip skills into ~/.codex/skills,
pipes prompts via stdin instead of passing as CLI args, filters out noisy
rollout stderr warnings, and extracts error/turn.failed messages from
JSONL output. Also broadens stale session detection for rollout path
errors. Claude-local adapter gets the same template vars (agentId,
companyId, runId) that codex-local already had.

PAPERCLIP_APPROVAL_ID, PAPERCLIP_APPROVAL_STATUS, and PAPERCLIP_LINKED_ISSUE_IDS

Extract claude-local and codex-local adapter code from cli/server/ui
into packages/adapters/ and packages/adapter-utils/. CLI, server, and
UI now import shared adapter logic instead of duplicating it. Removes
~1100 lines of duplicated code across packages. Register new packages
in pnpm workspace.
