docs: describe the rebuild task's two phases 

The task now refreshes the affected series after reassigning the
episodes, so the notes explain why both phases exist: reassigning an
episode does not create the season it now belongs to, and stopping after
the first phase leaves the previous seasons standing empty. Also notes
that progress runs 90% episodes then 10% seasons, so the run should be
allowed to reach 100%, and what a failed series refresh looks like.
